I'm trying to release my first app (so may be missing something obvious), but it passed review 4 days ago and has been 'Ready for distribution' since then without going live.
Is this normal, or am I missing some step I have to take?
I saw another thread that mentioned setting pricing, but I've done that.

Did you ask for automatic publication when submitting, or did you ask for manual publication ?

I just submitted my today manually. will let you know if my app gets published to the App Store in the next day or so.

Same issue here. The app won't publish and there is no way of cancelling.
I assume by now you have solved this, Apple in their infinite wisdom have changed ready for sale to ready for distribution. I had this when my app passed app review only to be removed from sale due to some issue with pricing and availability.
It done. Create a new version, with a plus button next to the iOS App.
