Go to developer.apple.com/ click 'Account' bar, login with your Apple ID and password. After you login, you will see text about Agreement on the top of the page. Click link and read through agreement, then sign it if you agree.

You can apply here: https://developer.apple.com/programs/universal/ . And remember to read "Eligibility" column. Also reading 'Universal App Quick Start Program terms and conditions' (on the same web page) is highly recommended.
