Question regarding completing U.S. Tax Forms

Hello everyone,

I am launching an App very soon with multiple in-app purchase options. There will likely be 9 options in total. I am just now setting up the merchant/bank account and I was shown that I am required to complete specific tax forms.

My company is a Delaware LLC. I am the sole member of this LLC. Am I correct in assuming that I am a "disregarded" entity? Also, when I am filling out the "Form W-9", whose name should I put in the first section "Name (as shown in your income tax return)"? Since I am a single member LLC, should this name be my actual name, and then in the second field, "Business Name", I should put the company name that I signed up with via Apple Developer?

Also, for the address field, should it be my company address or my actual address for mailing purposes? How will I receive any mail from Apple? Many thanks in advance for the help.

The problem is that LINE 1 on the W9 form on Apple's Agreements page is pre-populated with the LLC's name and doesn't allow modification for the case where an LLC is considered a "disregarded" by the government and therefore the owner MUST put their personal name down on Line 1.
