it really weird. I can't add device because it is grayed out and can't add more uuid. I checked Continue button but it says as first pic, I can add more 45 iPhone but + button is still grayed out

You need to go through the annual device reset process first before you can add any new devices. As the warning in the second screenshot shows, please click "Continue" to confirm if all the current registered devices are being used or if they are not, you can decide which ones to keep and which ones to remove.
