We noticed that your app or its metadata includes irrelevant references to your app’s development process.

Specifically, your screenshots contain "Debug" in the upper right corner.

Since apps on the App Store should be ready for consumers, including references to the development process in your app or metadata is not permitted.

For example, an app for a restaurant should offer more functionality beyond a standard mobile website that include its phone number, address, and hours. You should instead include functionality like table reservations, mobile ordering, etc. that do not currently exist on the mobile website.

Guideline 5.1.2 - Legal - Privacy - Data Use and Sharing

The app privacy information you provided in App Store Connect indicates you collect data in order to track the user, including Name, Phone Number, Search History, Emails or Text Messages, and Email Address. However, you do not use App Tracking Transparency to request the user's permission before tracking their activity.

Starting with iOS 14.5, apps on the App Store need to receive the user’s permission through the AppTrackingTransparency framework before collecting data used to track them. This requirement protects the privacy of App Store users.

Can you post a small (cropped) screenshot showing this "debug banner"? You're saying this isn't part of your app but was added by the system? Or if it is part of your app (such as to designate an internal debug build) then just remove it.

As for the other issues in the review, they seem pretty self-explanatory.
