Guided Access App Unavailable?

We have an app on hundreds of managed iPads (iOS 12) that have kiosk mode enabled and wifi turned off.

After a while (about 30 days or so) the app will refuse to run and the iPad will show a message that it is unavailable and lock up. The only fix seems to be to reset the entire device including the OS.

Any idea what might be causing this?
