IOHIDRequestAccess seems to break intermittently. I've been working on an app for the past year and I have to periodically restart the machine to get things working again. It seems to happen sometimes when I'm performing new builds. After going through long trouble shooting like restarting the app, restarting the terminal that launches the app, removing and readding the apps Input Monitoring permisions, I've come to the conclusion that macos is just broken. So far a restart has immediately fixed it every time. I'd be interested to learn how macos actually adds a binary to the permissions list. It doesn't seem to be by a hash but whatever it is I assume there is some cache that is bugging out.
