I was able to get the app to launch on the problem device by having the user delete the app, wipe the device, and restore from backup. The user restored the device from an iCloud backup and was then able to download and install the app. The Unable to Verify App error no longer shows up when launching the app.

It does not look like anyone has solved the issue. We are using an MDM and according to apple "If you install an app by Mobile Device Management (MDM), trust is automatically established." ( https://support.apple.com/en-us/HT204460 ) but that is not the case.
It appears that after adding display-image and full-size-image in the manifest.plist file the OTA install works.
