Getting a similar issue right now. After the first "Hey Siri", carplay stops receiving new Siri commands and apps start going crazy.

Good news! The issue seems to have been fixed in Beta 8. FINALLY!

For the second part on arbitrary local file, does it include assetURL obtained from MPMediaItem?
I've been facing a similar issue but with error code -54 when I do "file = try AVAudioFile(forReading: url!)".
