I am also curretly having the same issue after updating to IOS18. Whether wired or wireless sound quality is poof and very mono. At some point it corrects itself to go back to mono again. Making phone calls then going back to music doesnt resolve it. Bluetooth works fine
Same issue with my apple car play. iPhone 14 pro on iOS 18 Once conected via USC on lightning sound quality is very mono. All the sudden (random) it is back to normal before it goes to mono again.
Music also stops for a second or two when locking and screen or unlocking. Very annoying.
Any updates / bug fixes on iOS 18.1 planned?

Same issue 14 pro max! When plugged in initially it’s fine as soon as I pick it up it goes mono and says that way. Complete and horrible miss on your part Apple!! This needs addressed ASAP!
