Update: We requested and got family-controls(Distribution) entitlement for our extensions too, which enabled us to release it on TestFlight for testing. The performance of weekly reports have definitely improved but the underlying issue of getting a blank report (Like you said, 2 out of 10 times) still exists. Still not sure why this happens.

Yes, you can perform center.startMonitoring() from the main app too. As long as you have the event, activity and schedule there. Just make sure the apps/categories are selected for that event before monitoring.
You can use the DeviceActivityMonitor Extension to get the threshold of an event
@_lilpit thanks for the explanation on why Darwin notification is working only in debug mode. We have moved our shielding logic into the monitor extension and it works as expected. Thanks for responding
Yes, reinstalling the app and granting the permissions again worked for us as well. We're still facing this issue after 6 months, which means the issue is still persistent in newer versions of iOS.
