I am deploying an enterprise app using Microsoft Intune. I'm able to push the app without problems, but when I try to run it the user is presented with a warning that the enterprise developer is untrusted.

Documentation says that apps pushed with MDM don't require that users trust the enterprise developer.

Do I have to push the certificate out or is there some other undocumented step?

Microsoft's fingers are pointed firmly in the direction of Apple on this.

  Yep. They all either say how to manually trust the app or say that MDM makes it so that manually trusting isn't required.
  
  We don't want to manually trust the app and we have MDM but manual trust is still being required.

    Then I'd go back to your MDM implementation and return finger fire directly at MS.

      Apple Developer Support (whom I spent hours trying to talk to) are now saying that our enterprise account needs to be enabled for MDM vendor. Who knows if that will work or not.

If you're using an MDM, ensure that the restriction policy does not block "Allow trusting new enterprise app authors (iOS 9.0 and later)".
