Hello there. What did you ended up doing? What we did in our app was a static page like your number 3 but without a button. It said that this app cannot run without camera permission and if they want to use it please give permission in app settings. Maybe a button to open system settings would be helpful. But we let the user do it themselves..
From my experience iOS doesn't even allow to ask multiple times for permission. At least that's what I remember from iOS 12.
