This does work, but I have not been able to restore the Activity app even with this method. Every other app is resolved except for that one.

You have to download the IPSW and restore the phone using it. OTA update is when you update your phone from within Settings --> General --> Software update. This will result in this problem every time (at least for me anyway)

I have this issue as well. My first upgrade resulted in a lot of the first party apps showing up with the icloud icon and unable to be installed from App Store. The only way I was able to resolve is to restore using the IPSW (save for Activity App). An upgrade with the profile installed results in this problem every time for me. I have restored the iPhone several times using the IPSW, both B1 and B2 but unable to bring Activity app back.

You need to have Xcode 11 beta installed if you want to read your iPhone or iPad with IOS 13 betas on Mojave.
